State of Hawaii Department of Health regulations for post-secondary institutions require students to submit proof of health clearance for tuberculosis (TB) and measles. These clearances must be submitted to the Admissions and Records Office (Hale Alaka’i, Room 112) prior to registering for classes.
TB Clearance
The certificate of TB clearance may be issued by the Hawaii Department of Health or a U.S. licensed MD, DO, APRN or PA and must be dated within 12 months prior to the term of entrance.. The certificate must include:
- The dates of administration and reading of the Mantoux skin Test (PPD).
- The measurement in millimeters of the induration (raised skin reaction).
- The signature or stamp of the MD, DO, APRN, PA, or clinic.
Where can I get a TB test? TB skin tests are provided free of charge at the Windward Community Health Center next to WCC from 1:30 to 3:30 p.m. every Tuesday. You must then return to the clinic the following Thursday between 1:30 to 3:30 p.m. for your results. For more information, please call 233-5450.
Exception: Students who previously attended another college/university in the State of Hawaii. These students will not be required to take another test if they can present the original clearance document used to enroll at their previous institution. If you do not have your original certificate, please contact the Windward Community Health Center (see above).
Measles Clearance
Two doses of Measles vaccine are required, with at least one of the two being Measles-Mumps-Rubella (MMR) vaccine.
How do I provide measles clearance? If you do not qualify for any of the above exemptions, you are required to submit proof of immunization and/or disease history with one of the following:
- A record of immunization, signed or stamped by a US licensed MD, DO, APRN, PA or clinic.
- A blood test (serology report) showing laboratory evidence of immunity to measles, mumps, and rubella signed by a US licensed MD, DO, APRN, PA or clinic.
Exemption: Students born before 1957.
Where can I get a measles clearance? If you do not have a physician, a suggested option for obtaining measles vaccinations is the University of Hawaii at Manoa Student Health Services (956-8965).
If you need any additional information regarding health clearances, please call the Office of Admissions and Records at (808) 235-7432. |
