Financial Aid Forms

At times, the Financial Aid Office will need additional information to determine a student’s aid eligibility. Students are informed of any additional required documents through their myUH Portal and also through a mailed letter. If requested to, please download, complete, and submit the following forms to the Financial Aid Office.

• Verification Worksheet 2009-2010 for Dependent (PDF) - also requires parents' information
• Verification Worksheet 2009-2010 for Independent (PDF)
• Military Allowance Information Form 2009-2010 (Word Document)
